Eligibility
Full
time, active employees working at least 30 hours per week are eligible on
the first day of the month following 30 days of employment. Part
time employees working at least 20 hours per week are also eligible.
What is my coverage
Your
beneficiary will receive a benefit of $40,000. If you die in an accident
there will be an additional $40,000 benefit.
Additional Life Coverage
You have the option to purchase Voluntary Life and AD&D insurance for you and your dependents. Employes can elect up to 5 times your annual salary, not to exceed $500,000. You may purchase coverage for your spouse (up to 50% of employee amount) and children ($10,000 benefit). Proof of "good health" may be required.

You
and Rehab Solutions share in the cost of this coverage. Click on the link
below to view these costs. You will need a User Id, which will always be rehab,
and the monthly password. You can
get the current password from Human Resources.
